This form is for authorized institutional representatives, qualified technical collaborators and potential investment or development partners. Provide accurate, verifiable information. Do not include patient health records, passports, nursing candidates’ personal data, confidential designs or other sensitive documents.
There are 58 identity, evidence-readiness and tailored technical questions across 6 short questionnaire stages, followed by email verification. Document readiness declarations are mandatory. Once the verified enquiry is securely recorded in MHC FEL, upload the required evidence using the private document stage. Full ID documents must be supplied only through that private channel. Microsoft 365 email is used for notifications; online case tracking is not yet active.
